On-line Patient Form Pulled for Lack of Security

The University of Pittsburgh Medical Center has removed from its Web site online forms used to collect patient information, including names, Social Security numbers and prescriptions, after realizing the forms lacked basic security protections. Experts expressed surprise that UPMC, known for its expertise in IT, did not take simple precautions.
